The Wellness Plan Medical Centers strives to stay connected with its patients and the community with a variety of events and activities. From family literacy programs to informative webinars to Family Fun Days, we do what we can to make healthcare both fun and informative. Our community outreach team offers programming that covers health education, including health promotion and disease prevention. Other educational sessions that TWP offers are alcohol, tobacco, and drug prevention, and chronic disease management sessions. In addition, we partner with community agencies that specialize in HIV, AIDS and STI prevention programs, nutritional programs, and physical activities.
Our community outreach team distributes information materials about our services to local residents. Their goal is to serve as a liaison between patients and organizations that can provide services to our patients beyond TWP’s services. Through these efforts, TWP will assist in making appointments, and gathering information for patients from. In addition, TWP can aid in enrollment of community and governmental programs, like Medicaid or other MI Bridges programs.
MI Care Program
The MI Care program is a health benefits program for Medicaid, Healthy Michigan Plan and MI Care Child enrollees. The program arranges the health and social needs of the patient, similar to having a personal health coach. To qualify, a patient must have:
- Depression and/or anxiety
- Diabetes, heart disease, hypertension, Chronic Obstructive Pulmonary Disease (COPD), or asthma
Our MI Care Team provides care management that caters to the patient’s specific needs. We provide the resources and support needed for patients to live their healthiest life. The team includes your medical provider, a member of the integrated behavioral health program, and a community health outreach specialist. A patients participation is voluntary.
