Introducing The Wellness Plan’s Patient Portal
The Wellness Plan is committed to providing quality healthcare to our patients. The health and wellness of our patients is our primary focus and why we are excited to introduce our new Patient Portal.
The Patient Portal is an innovative and user friendly tool that will allow patients to: access to their health information, update personal and insurance information, schedule appointments, request prescription medication refills and securely communicate with your Wellness Plan provider.
Beginning in the Fall of 2018, we will be offering the following services through our patientportal:
- Updating your Personal and Insurance information
- Requesting an Appointment with your Provider
- Requesting Prescription Refills for your Medications
